How do I know these fit my shaver?

By the code pattern rather than the marketing name. Philips states S1XXX, S3XXX and S54XX. The model code is printed on the shaver body and in its manual — the S3145 and S5465 reviewed on this site both match, and the X3053 does not, despite being sold as a Series 3000X.

Do I have to replace all three at once?

They are sold and specified as a set of three, and the reminder icon refers to the head assembly rather than to one cutter. Philips does not describe replacing them individually.
