With a guard fitted, always — the guard holds the blade off the skin, and a nicked mole almost always starts with a bare blade run over ground the user could not see.
This is one of the few places where a specification directly answers a risk.
A guard. Even the shortest comb keeps the cutting edge clear of raised skin. A tool used bare passes directly over it.
A skin guard. Braun publishes SkinGuard on the BG3530, the BG5555 and the AIO7565; Philips publishes a hypoallergenic foil on both its body groomers and a 4D contour system on the top one; and the Philips OneBlade at $95 ships a skin guard as a separate click-on part. These are built for uneven ground, and the OneBlade is the cheapest of them.
Slow, short strokes, and your free hand flattening the skin around rather than over the spot.
Know where they are before you start. A mole that is invisible under a beard is invisible to you and not to the blade.
If a raised mole sits exactly on a line you trim regularly, leaving the hair slightly longer over it is a reasonable answer, and no one will see the difference.
Anything that bleeds, changes or does not heal is a matter for a doctor rather than for a page about trimmers.
